File a Claim

If you have any questions or need help filling out the Claim Form, please send an email to info@ncaavolunteercoachlawsuit.com.

The Settlement Administrator may request supporting documentation in addition to the documentation and information requested in the Claim Form. The Settlement Administrator may reject a claim if the Class Member does not provide all requested documentation in a timely manner.

Claim Forms must be postmarked (if mailed) or received (if submitted online) no later than June 2, 2026.

To file your claim by mail, download a copy of the claim form below.

If you timely submitted a Claim Form, no further action is required at this time. If additional information is needed to process your claim, the Settlement Administrator will contact you.

You may be contacted by companies or individuals offering to assist with filing your claim for a fee. They are not affiliated with the Court, Plaintiffs’ Co-Lead Counsel or the Settlement Administrator.
